Private ADHD Titration

Titration

After your assessment, you will be informed if medication would be beneficial for your situation. The discussion will take place at your follow-up appointment (which may be scheduled in person, via video link or by phone). Your doctor will go over the different options for medication available. If you decide to start taking medication, we will offer you the option of titrating your dose to maximize the benefits/side effects balance for you.

This is a process which can take several weeks. It involves adjusting the dosage of your child's medication gradually until the proper balance is achieved. It is not uncommon to have to adjust up to three different medications until you discover the one that best fits your child's ADHD. https://www.iampsychiatry.uk/private-adult-adhd-titration/ for parents to take part in this process of titration and keep a close watchful eye on their child's behavior so that they can inform the doctor whether the medication is helping or causing adverse effects.

Unless you have decided to return to NHS treatment and have decided to return to NHS care, your medication will be provided with a private prescription which you'll need to pay for at the pharmacy (this is not included in the consultation fee). You'll be required to attend every six months to check your medication. This includes pulse and blood pressure readings. Annual reviews are also required.

Follow-up appointments

Once the assessment has been completed, you will be able to book follow up appointments with the psychiatrist, as appropriate. The appointment usually takes place through a video call using zoom however in some instances it might be possible to schedule an audio only consultation (again at an additional cost).

We will then discuss the options for treatment and then decide on the most appropriate, customized ADHD medication adjustment. This will include a review and discussion of your medication dose and any side effects that you may be experiencing. These titration sessions cost PS150 for face-to-face sessions and PS60 via telephone or video consultation. The cost of medication is additional and are paid directly to the pharmacy.

The psychiatrist will ask questions regarding your family history and the impact of your ADHD symptoms on your life, and any mental health issues you may be experiencing. This is because ADHD can be a genetic condition and it is essential to look into any family history that may suggest that you are at risk of developing the disorder.

During this examination, it is essential that you are honest and open with the doctor, as they will use the information you provide to determine whether you are able to meet the diagnostic criteria for ADHD. This is a crucial step in the process because if you don't do it, it could result in an incorrect diagnosis and therefore inappropriate prescribing of medication.

If you have decided that you would like to receive treatment for your ADHD We will set up a follow-up appointment with one of our psychiatrists that specialize in this area. This can be scheduled in the wake of your diagnosis if want to start treatment right away, or within two weeks if you prefer to take time to think about the implications of your diagnosis and the possibilities available to you.

If you are unsure of what you would prefer, we suggest that you talk with your GP prior to scheduling your private assessment as some providers have different policies regarding whether they require an GP referral letter or not. Make sure that your GP accepts a Shared Care Agreement, which means that you will be referred to them once you leave the clinic for prescription medication.

Medication

For many people who seek an official diagnosis of ADHD, medication is an important part of their journey. It can be difficult to get a prescription from a psychiatrist on the NHS. NHS waiting lists can be notoriously long, and GPs might be reluctant to refer patients to specialist services.

Inquiring your GP to refer through Right to Choose is one way to avoid this (see psychiatry - UK and ADHD 360). You can select a provider who offers an assessment on your own and the titration. This will help you reduce time and be more convenient, especially in the event of a hectic schedule.

Once you have been referred through this route, your chosen clinician will evaluate you and determine whether or not you need to be titrated on medication. You will be asked to fill out forms that are different for each provider, but the most common ones are self-report questionnaires and medical histories from childhood to adulthood. These forms will allow your doctor to determine if there are any reasons that suggest you shouldn't take ADHD medication, when they've been prescribed.

If you are being titrated the medication will consist of stimulants and nonstimulant medicines. They will be carefully tested and monitored to determine the most suitable combination for you, taking into account any existing conditions you have. The titration procedure can be long and may require a number of additional appointments before you are stable.

Depending on the type of medication you are taking, follow up appointments will be either monthly or quarterly. This will involve your clinician checking to see if you are stable on the medication, noting any adverse effects you've experienced, and making any adjustments needed to ensure you are on a dose that is safe. You are required to attend an annual review as well as a routine health examination which includes blood pressure, pulse, and weight measurements.

For some individuals, it may be that they are content with their medication and don't need any further discussion on their titration. If you need to discuss your titration plan or treatment plan further it is likely that you will be referred back to the private doctor that performed your initial evaluation. If this happens, the titration appointment will be part of their consultation fee and they will receive their medication on private prescription (see Consultation fees above). This could cost more than an NHS prescription because the pharmacy will charge for the medication.
